What the inspector wrote
On 7/7/25 at 8:45AM, Licensing Program Analysts (LPAs) Angela Luz and Andy Yang met with Infant Site Director Audry Carbajal for a Required - 3 year inspection
. Present at the time of the inspection were 9 staff & 27 children. The facility is within ratio & capacity compliance today. LPAs provided Infant Site Director the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates Monday-Friday 7AM-6PM in classrooms 1, 2, 7, 8, and 13. This facility also has school age (434408756) and preschool (434408755) components, which are physically separate from the infant component.
Classrooms:
All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children’s belongings. LPAs observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and fully charged fire extinguishers that are accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms. The isolation area for sick children is the office.
LPAs observed sufficient age-appropriate materials, toys, and play equipment in the facility. Toys are safe and do not have sharp edges or small parts that may pose a choking hazard. The floors are clean and free of tripping hazards and waste containers have tight fitting lids.
LPAs observed the changing station to be set-up and maintained according to Title 22 regulations. Infant changing tables were observed to be padded, within arm’s reach of a sink, in good repair and safe condition.
Infant Safe Sleep:
LPAs observed cribs used by the infants are free from loose articles, covered with a fitted sheet, and there are no objects hanging above or attached to the crib. Infant Site Director states infants are placed to sleep on their backs. 15-minute sleep checks are conducted and documented. Swaddles, including sleep sacks, are not used. Bedding for non-walking rooms are provided by the facility and washed daily. Bedding for walking rooms are provided by families and washed weekly.
Food Service Areas:
All children’s individual food items from home are labeled and properly stored. All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Snack menu is posted at least one week in advance, available for review, & dated. Solid waste bins are equipped with tight fitting lids.
Outdoor Play Area:
The outdoor play area is fully fenced and there is an ample amount of shade. All equipment and surfaces are free from hazards. There are age-appropriate outdoor toys and materials for the children. There are no bodies of water, or free-standing water accessible to children.
Records:
All individuals subject to criminal record review have a clearance and have been associated to the facility. LPAs reviewed the facility roster & personnel report. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification.
Infant Site Director’s CPR/First-Aid Certificate is current and expires on 1/26. Infant Site Director's Mandated Reporter training is current and expires 12/25. Infant Site Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years. The center is in compliance with sign in and out procedure. Fire/Disaster drills are recorded and performed at least every six months, last drill was on 4/7/25. Infant Site Director states there are no firearms on the premises. All required documents are posted in a publicly accessible area.
LPA Luz reviewed 5 children files and 6 staff files during today's inspection for required documents. Each infant under 12 months of age has an Individual Infant Sleeping Plan maintained in the file signed and dated by the infant’s authorized representative. Infant Needs and Services Plans for each infant are updated for each infant every 3 months. All children and staff files reviewed today were complete.

Criminal Record Clearance
: Infant Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Lead Testing:
This facility tested for lead in January 2023.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Safe Sleep:
LPA Luz discussed the safe sleep regulations with Infant Site Director and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed Infant Site Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS):
This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children’s, personnel, and administrative records. MyChildCarePlan.org:
Infant Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Unusual Incident/Student Injury Report:
Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 calendar days.
No deficiencies are issued today.
Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days.
