What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required 1- Year inspection. LPA met with Director Nakia Centeno and explained the reason for the inspection. Upon arrival, there were no school-age children present. At 1PM, 11 school-age children arrived. There were 11 children with at least 2 staff. There is an area to post required postings, such as license, parent's rights, and car seat law. There is a working phone at the facility. Facility uses electronic sign in/sign out. All children were signed in by staff. LPA toured the inside and outside of the facility. All disinfectant, cleaning supplies, and other items that could pose a risk to children were observed to be inaccessible. Bathroom for children were observed to be clean. Facility also uses the bathroom next to Room 2. There are toys and equipment for children that are age-appropriate. There is a fully charged fire extinguisher, smoke detector, and carbon monoxide. The last disaster drill was conducted on 04/2022. Director stated that there are no weapons, such as firearms, stored on the premise. Facility has a waiver to share outdoor space with the preschool program. The area is fenced. There is resilient material around play structure. Shaded rest area is provided through trees, building overhang, and canopy. There were no bodies of water observed during today's inspection. Facility provides snacks to children. Menu was observed to be posted. There is kitchen to store and prepare food. Drinking water is provided through water pitchers and cups. ---------------------continues on 809 dated 09/21/2022 page 2--------------------- ----------------continuation of 809 dated 09/21/2022 page 1---------------------- Facility does transport children. Staff who transport children completed CPR/1st Aid through Medic 1st Aid, which did not have proof that it was EMSA approved. Director understands that CPR/1st Aid needs to be EMSA approved or completed through American Heart Association or American Red Cross. Staff who transports children have a valid driver's license. This facility does not currently provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. Five (5) children's files were reviewed during today's inspection. The records reviewed include but not limited to admission agreement. Two (2) staff file was reviewed during today's inspection. The records reviewed include but not limited to education credit and Mandated Reporter training. S-1 completed Mandated Reporter training on 09/15/2019. LPA discussed with Director that training required renewal every two years. S-2 is missing LIC 9108 and needs to sign the second page of the LIC 508. There is a staff on the preschool program that has a valid CPR/1st Aid, which expires on 03/2024. All staff present have cleared fingerprints. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. As a result of this inspection, no deficiencies were issued. Exit interview conducted and report was reviewed with Director Nakia Centeno. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
