What the inspector wrote

On 08/28/25 at 8:35 AM, Licensing Program Analyst (LPA) Jaime Gonzales conducted an unannounced annual random inspection. LPA was granted access to the home by the licensee Farahnaz Akhtariaraghi. Upon arrival there were one infant and four preschoolers. Others in the home during the time of inspection were the licensee, assistant and Ex- Husband. During the inspection additional children that arrived by 10:55AM were one infant and two preschoolers arrived 10:55AM. The home was toured to conduct a Health and Safety Inspection. LPA observed all required postings in the living room area that is used for the daycare children. Days and hours of operation are from Mon.-Fri. 6:30AM-6PM. The ages of the children to be served are 0-5 years old. The home is neat and clean with heating and ventilation for safety and comfort. The off-limit areas include: Garage, Master bedroom, Master bathroom, one bedroom and right side of the yard when facing the home. Licensee stated that kitchen and dining room is off limits but children will walk through the area to get to the bathroom and are supervised when walking from the day care area to the restroom. Off-limit areas are inaccessible by closed and/or locked doors and with visual supervision. There are ample age-appropriate toys that appear to be safe and in good condition. LPA observed a water fountain located in the backyard that does not have water and is unplugged. LPA discussed that if it rained licensee would need to check if water was in the fountain and will need to drain it. LPA observed a wood piece on the bottom of the fence on the left side yard of the home was coming off and has a two-inch gap between the top and bottom of the fence. Licensee will ask ex-husband to fix the fence and children will not use the side yard until it is fixed. Licensee stated when children are sick they are placed in a play yard and isolated in the dining room where they are supervised but away from other children. LPA observed poisons stored in a container that is high in the cabinet and is not accessible to children. LPA reminded Licensee that smoking, sleep sacks, baby walkers, and similar items are not allowed in Family Child Care Homes. Continued on Page 2... The home has a fully charged (2A10BC) fire extinguisher. LPA observed a inoperable Smoke detector/carbon monoxide detector and a new operable smoke/carbon monoxide detector was installed during the time of inspection. Licensee confirmed she has a working telephone. The Licensee CPR and First Aid certificate is current and expires (04/20/2026). Licensee\342\200\231s Mandated Reporter Training for Child Care Providers expired on (07/11/2025) and licensee will sign up to complete the course during nap time today on 08/28/2025. Licensee was reminded of Mandated Reporter Training (AB1207) and CPR/First aid certifications needs to be renewed every two years. A copy of the licensee\342\200\231s immunization is on file. Per Licensee, there are no firearms in the home. The Licensee conducts and documents fire and disaster drills at least every six months and was last conducted on 07/07/2025. Personnel records are available for review and assistant is missing immunization records. LPA reviewed four children's files and observed that C2 immunization records were outdated and C3 was missing immunization records. LPA did not observe documentation of 15-minute checks while sleeping for C1. All cribs or play yards meet the safety standards. The mattresses in the crib or play yards are firm and covered with a fitted sheet that is appropriate, fits tightly, and overlaps the underside of the mattresses. Each infant\342\200\231s bedding is used for them only and cleaned weekly or before use by another infant. LPA observed mesh bumpers around the cribs, no children were in the crib at the time of inspection and licensee removed them during the inspection. LPA and licensee reviewed the infant sleep regulation together during todays inspection and a copy was left at the facility. Forms of discipline to be used by Licensee are redirecting and talking with the child . Licensee understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, interference with eating, sleeping, intimidation, or other actions of a punitive nature. Children are treated with dignity, and receive safe, healthful, and comfortable accommodations. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Continued on Page 3... During the exit interview, confirmed that there are no Registered Sex Offenders living in the facility. LPA discussed the safe sleep regulations with licensee, Farahnaz Akhtariaraghi and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee,Farahnaz Akhtariaraghi of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. A notice of site visit was given to licensee, Farahnaz Akhtariaraghi and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days .
