Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required 1 Year inspection. LPA met with Principal/Director Violet Seryani and explained the reason for the inspection. Present during today's inspection were 25 children and at least five staff. LPA reviewed the electronic sign in/out. LPA toured the inside and outside of the facility. The center was observed to be clean and free of tripping hazard. Cleaning supplies, disinfectant, and other items that could pose a risk to children were observed to be inaccessible to children. Toys and equipment were observed age-appropriate. There is fully charged fire extinguisher, smoke detector, and carbon monoxide detector. At 3:50PM, LPA observed that a child went to the restroom. LPA also observed at 3:51PM that a child went to the other restroom. There were staff in the room. LPA observed that the staff moved to the door where they can supervise the children in the room and the chid in the restroom. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). ------------------continues on 809 dated 09/21/2023 page 2------------------------- ---------------continuation of 809 dated 09/21/2023 page 1----------------------- LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. The outdoor activity area is fenced. Areas around playstructure has resilient material. Shaded rest area is provided through umbrella and building overhang. There were no bodies of water observed during today's inspection. All meals and snacks are either prepared and brought from home or through Choice Lunch. Drinking water is provided through individual water bottles or Brita filter and cups. A copy of the facility roster was obtained. LPA reviewed the physician's report and immunization records during today's inspection. LPA was unable to review Licensing form/documents during today's inspection. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The records reviewed included but not limited to education credit, Mandated Reporter training, and Health Screening. Principal Violet's CPR/1st Aid expired on 09/2022. Principal stated that they have a CPR/1st Aid training scheduled for next month. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Principal Violet Seryani was informed that informed that the Annual Inspection will continue and be completed at a later date. As a result of this inspection, deficiencies were issued. Exit interview conducted and report was reviewed with Principal Violet Seryani. A notice of site visit has been issued and must remain posted for 30 days.
