APPLICATION FOR INITIAL LICENSE
Application for Initial License - Evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care. This requirement is not met as evidenced by: Per review of records, H-1 is missing TB test in her file. This poses a potential risk to the health and safety of children in care.

STAFFING RATIO AND CAPACITY
STAFFING RATIO & CAPACITY - A large family day care home may provide care for more than 12 children and up to and including 14 children, if at least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age. This requirement is not met as evidenced by: LPAs observed 13 children present in the home including three (3) infants and 10 preschool age. This poses an immediate risk to the health and safety to children in care.

Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles.
