Correction check·February 13, 2026

On 02/13/2026 at 9:30am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Plan of Correction Visit and met with site director, Esther Rosenblat. Also present at the time of today's inspection is 2 staff and 10 children. The purpose of today's inspection is to follow-up on the Plan of Corrections (POC) for deficiencies cited during an annual inspection on 01/29/2026. These POCs were due to be completed no later than 02/09/2026. On 01/29/2026, the facility was cited 3 Type B Violations related to record keeping. These citations included personnel records being inaccessible and unavailable for review, children's records being inaccessible and unavailable for review, and the 15-minute checks on sleeping infants being performed without documentation on sleep logs. During today's visit, LPA Uribe inspected personnel, children's records, and sleep logs. LPA Uribe has verified that the director has fulfilled the POC requirement and all three deficiencies issued on 01/29/26 are being cleared today. Upon further review of these records, the personnel and children's files were found to have incomplete or missing documentation that is required to be kept on file. Two Type B Violations are issued today for incomplete personnel records and incomplete children's records. See attached LIC 809D page for information of deficiencies cited today. Notice of site visit was given and must remain posted for 30 days. Failure to follow posting requirements will result in the immediate civil penalty of $100.
