REPORTING REQUIREMENTS

Reporting Requirements 101212(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...... This requirement is not met as evidenced by, as per the Licensee’s reporting on Friday, August 2, 2024, 6:32 PM via email to LPA Estoesta. Incident date 7/30/2024. This poses a potential risk to the health, safety or personal rights to children in care.

101227 Food Services (a) (18) All kitchen, food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin...... This requirement is not met as evidenced by, at LPA observed rodent droppings (poop) in the locked Cleaning Supply Storage (Off Limit Area) where the heating, ventilation, and air conditioning (HVAC) equipment system is located. The locked Cleaning Supply Storage (Off Limit Area) is in the children’s and staff bathroom. This poses a potential risk to the health, safety or personal rights to children in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

No child(ren) shall be left without the supervision,including visual supervision, of a teacher at any time, except asspecified in sections 101216.2 (e)(1) and 101230 (c)(1)....... On 05/23/2023 at approximately 4:26 PM, C1 was found by P1 outside at the sidewalk of the side parking lot adjacent from the outdoor play exit door without supervision of a teacher, which was an immediate risk to the health, safety, or personal rights of C1.

REPORTING REQUIREMENTS

Based on LPA's Record Review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Staff 4 was hired a new Director in March 2022 and there were no reporting submitted to the Regional Office.
