Licensing Program Analyst (LPA) Jaime Gonzales was greeted and granted access to the facility by Site Director/Licensee Adidi Bouzida. LPA toured both indoors and outdoors during the inspection. Upon arrival, LPA completed a census head count in two preschool classrooms. Classroom C had 9 preschools and one staff. Classroom B1 and B2 is currently used as one space due to no enrollment in their 4-5 year old. Classroom B1 and B2 had 7 preschoolers and one staff. LPA observed all required postings at the entrance of the facility. Hours of operation for the facility are Monday \342\200\223 Friday, 07:00AM-6:00PM. The classrooms have age-appropriate materials, toys, equipment, & adequate storage for children\342\200\231s belongings. The children's napping equipment were observed to be stored in a healthful manner. Bedding is provided by the parents and is washed weekly or as needed. LPA observed a fully charged 3A40BC fire extinguisher, functioning smoke and carbon monoxide detectors. The isolation area for sick children is in the office or in the classroom on a cot in the classroom. LPA observed that the classroom temperature in B1 and B2 was 63 degrees at the start of the inspection at 8:30AM. During the inspection, Site Director turned up the heater and turned on space heater in the classroom. LPA went to observe it a second time with adult staff one thermometer read 67 degrees in classrooms B1 and B2. LPA went to observe a third time during the inspection with Site Director/Licensee and classroom B1 and B2 thermometer read 71 degrees. Facility offers full day program and provides midmorning snack and afternoon snack to children. Lunch is provided by the parents. The snack menu is in writing and posted at least one week in advance, accessible to authorized representatives. Drinking water is readily available for children by water bottles from home labeled with the children\342\200\231s names. Water bottles are filled throughout the day by water pitchers. The outdoor play areas are fully fenced. Shade is provided by building overhang and trees. There are no bodies of water on the premises. There are age-appropriate outdoor toys and materials for the children. LPA observed a climbing structure that had sufficient resilient material. Director state that the resilient material is refilled once a year. Director states that the facility does not provide transportation for preschool children. Continued on Page 2... At least one staff member has a current Pediatric CPR/First-Aid certification. LPA reviewed sign-in/out sheets, facility roster (LIC9040), and fire/disaster drill log during today\342\200\231s inspection. The last Fire/disaster drill was conducted on 10/16/2025. LPA reviewed five random children files during today's inspection and all files were complete. LPA reviewed three staff files for the following records: Health Screening Report (LIC 503), Immunization records, TB Test, Statement Acknowledging Requirement to Report Child Abuse (LIC 9108), Employee rights (LIC 9052), staff qualifications, Mandated Reporter Training and all files were complete. Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter Training every two years. Licensee, Adidi Bouzida was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Continued on Page 3... Licensee, Adidi Bouzida was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the licensee, Adidi Bouzida confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
