What the inspector wrote

On 01/15/2026, Licensing Program Analysts (LPAs) Jennifer "Jen" Beehler and Janette Cruz conducted an unannounced Case Management and met with Jamie Ferguson, Director. Also present during today's inspection are Brittany Strange, Regional Director and Cathy Jelic, President/owner of Action Day Primary Plus. The purpose of today's inspection was to verify the capacity increase issued on October 24, 2017 by obtaining updated measurements for indoor and outdoor spaces. Upon arrival, LPAs were greeted by the Director and provided access. LPAs provided the reason for the visit. There were 36 infants present and 10 staff which is compliant with the ratio and capacity requirements. LPAs toured and re-measured the on-limits areas, classrooms and outdoor spaces at the facility. The facility has a total of 2 classrooms. Infant Classroom measurements are: The Indoor total measurements contain a total of {1,641.93} square feet, which can accommodate 47 infant children. Infant Outdoor measurements are: The Outdoor total measurements contain a total of {5506.83} square feet, which can accommodate 47 infants. There are 64 chairs, 8 tables, 150 cubbies, 48 cots, 1 potty chair, 4 sinks, 12 cribs and 4 changing tables available for the infants . Director was advised that there shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every 5 infants being toilet trained. During today's visit, the following documents were collected: Updated Lease agreement for the terms dates of August 1, 2025 - July 31, 2030 Updated LIC610 Dated 01/15/2026 LPAs notified facility representatives the following documents need to be updated and provided to the Department: LIC500 Personnel Report LIC999 Facility Sketch (with any changes to the room) must include a bird's eye view of the facility Check for $25.00 for capacity decrease Updated LIC200A Decrease of capacity shall apply upon the receipt of the above documents and the completion of the following: Approved fire clearance reflecting updated capacity Final management approval Exit interview conducted with Facility Representative, Jamie Ferguson, report was reviewed and provided along with appeal rights. No deficiencies were cited during today's inspection. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
