What the inspector wrote

On 9/27/2021 at 9:17am Licensing Program Analyst (LPA) Morgan Pringle met with Director Oksana Bazhenova for an Unannounced Annual Inspection . Three (3) classrooms, The Green Room, The Pink Room and The Blue Room, were toured for a health and safety inspection. Forty-three (43) preschool children and six (6) staff members were present during the inspection. The facility operates from 8:00am – 6:00pm. The facility has age appropriate materials in all classrooms that are observed to be clean and in good condition. The outdoor space has ample shade for the children and has proper materials for the children. All toxins, cleaning products, and hazardous materials were observed to be in inaccessible areas. All sinks and toilets were observed to be clean and in proper working order. The food preparation areas were observed to be clean and free from hazards. All knives and cleaning products in the food prep areas were made inaccessible to children in care. LPA did not observe any harmful or unattended bodies of water in or around the facility. The facility is operating within its licensed capacity and is in ratio. All proper postings including the menu were made visible to parents and visitors on the gate outside of the building. The fire/disaster drill log was complete with the last drill logged 9/22/2021. A physical census of the children and staff was taken and cross referenced with the sign-in and out log. LPA obtained all the children’s files and the staff files. All children’s files were complete. Four (4) teachers (T1, T2, T3 and T4) were missing a Mandated Reporter certificate of completion. One (1) teacher (T3) was missing LIC503: Health Screening Report (see LIC809-D). Continued on 809-C Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Personnel Roster must be properly maintained, and fire/disaster drill must be conducted every six (6) months and documented. Director was reminded that California Law requires all facilities to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC624). Incidents must be reported within 24 hours by phone, fax, or email. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov . Director was also informed that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com . Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
