What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Required -3 year inspection. LPA met with Licensee Laura Ana "Lori" Velasco and explained the reason for the inspection. Present during today's inspection were Licensee, two assistant, her adult son, her son's girlfriend, who helps with the family child care home, and 11 children, whom two were infant age. There is an area to post required postings, such as license and notification of parent's rights. The hours of operation Monday through Friday 7:30AM to 4:30PM. Licensee uses her cell phone. LPA toured the inside and outside of the home with Licensee. The off-limit areas of the home are the entire upstairs, laundry room, and garage. There are stairs in the home, which is barricaded. The fireplace is also barricaded. There are toys and equipment for children. Cleaning supplies, disinfectant, other items that could pose a risk to children were inaccessible. LPA reminded Licensee that blades for blenders need to inaccessible. There is a fully charged fire extinguisher, smoke detector, and carbon monoxide detector. Licensee stated that there are no weapons, such as a firearms, stored in the home. The backyard is used and is fenced. The off-limits outside is the left side of the yard. There are toys and equipment for children. LPA discussed with Licensee to check sandbox table and any containers that are cracked. There were no bodies of water observed during today's inspection. ------------------CONTINUES ON 809 DATED 12/07/2023 PAGE 2--------------- -------------CONTINUATION OF 809 DATED 12/07/2023 PAGE 1-------------- LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA provided Licensee with PIN 20-24-CCP. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. Plan for Providing IMS is in file. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/ . A copy of the facility roster was obtained. 10 children's files were reviewed during today's inspection. The records review ed include but not limited to immunization records and parent's rights. Licensee and her assistant's files were reviewed. The records reviewed include but not limited to Mandated Reporter Training and immunization records for measles, pertussis, and influenza. Licensee's CPR/1st Aid expires on 03/2024. One of assistant's CPR/1st Aid expires on 04/2025. LPA discussed with Licensee about obtaining a signed LIC 9149. Without this consent, the Licensee understands that she can only operate with a maximum capacity of 12 children and and updated license will be sent to reflect the change to capacity. -----------------CONTINUES ON 809 DATED 12/07/2023 PAGE 3--------------- -------------CONTINUATION OF 809 DATED 12/07/2023 PAGE 2--------------- The adults living in the home are Licensee, her adult son, and her son's girlfriend. Licensee also has one minor child and grandchild that lives in the home. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. During the exit interview, the Licensee Lori confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A updated lease agreement was submitted to Licensing during today's inspection; along with her son's and her son's daughter's TB test. Licensee will submit the following: - LIC 9149 - LIC 279 No deficiencies were issued as a result of this inspection. Exit interview conducted and report was reviewed with Licensee Lori Velasco. A notice of site visit has been issued and must remain posted for 30 days.
