What the inspector wrote

On 12/12/2025 at 12:10pm, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an unannounced case management inspection for the purpose of investigating a reported incident. LPA met with Director Kimberly Kostepen. Also present during the inspection were 39 fingerprint-cleared staff and 113 children in care. The facility is within ratio and in compliance with capacity regulations today. The facility was toured to conduct a Health and Safety Inspection. The incident occurred on 11/11/2025 and was self-reported to Licensing on 11/13/2025. (Director attempted to make an incident report on 11/12/2025.) Per Director, a child (C1) slipped in the classroom near the handwashing sink and sustained an injury that required medical attention. Parents were notified immediately following incident. An incident report was filled out, and a copy was provided to parents. During today’s inspection, LPA reviewed the above information with Director and conducted interview to obtain more information. Per Director, new anti-slip mats were added to the classroom as a safety precaution to prevent accidental slipping. LPA conducted interview with staff (S1) who witnessed the incident. Based on interviews, the incident was accidental and an isolated incident. LPA observed mats were in place in most preschool classrooms under the handwashing sinks. LPA tested the mats and identified some mats were not anti-slip. LPA shared observations with Director and advised anti-slip mats to be added to classrooms without mats or replace mats that were not anti-slip. No deficiencies were cited during today's inspection. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Kimberly Kostepen. Page 1 of 1.
