Inspector’s narrative

What the inspector wrote

During today\342\200\231s inspection, LPA conducted interview with Director and RP on site. Per Director, the staff who RP allegedly witnessed a child being force fed has been terminated. The termination was determined by various factors. LPA received a copy of the termination letter for verification. Per Director, meetings and training regarding infant care, including feeding, were held months prior to this incident. Resources were also shared via email. Copies of some meeting notes and resources were obtained. LPA also briefly observed the classroom where the incident occurred. Based on LPA\342\200\231s observations, interviews, and obtained documents, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, 101223(a)(3) is being cited on the attached LIC 9099D. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Senchal Rodriguez. Page 2 of 2. End of Report.
