What the inspector wrote

Licensing Program Analyst (LPA) Melanie Otsuji conducted an unannounced REQUIRED 1 YEAR INSPECTION & CAPACITY DECREASE on today's date, 9/16/2021. LPA was met by Director, Ollia Yenikomshian and Assistant Director, Rebecca Pheister. Present during today's visit were 58 preschool aged children and 13 staff members. The center has submitted an application for a CAPACITY DECREASE to the PRESCHOOL COMPONENT . Also on site is an infant component with the option to include toddler option (CAPACITY 64) operating out of 5 rooms (Peek A Moo, Mary Had a Little Lamb, 3 Little Pigs, Pokey Little Puppy and Pat the Pony). PRESCHOOL COMPONENT is currently requesting to decrease from 92 preschoolers to 76 preschoolers. A health and safety inspection was conducted inside and outside. Facility days and hours of operation are Monday through Friday 6:30AM - 6:30PM. The facility measurements have not changed and were completed on a previous visit.. Measurements are follows: INDOORS: 3936.43 SQUARE FEET = 112 CHILDREN OUTDOORS: 8073.03 SQUARE FEET = 108 CHILDREN The center has obtained an approved fire clearance from Santa Clara County Fire Department for 140 children which includes 76 preschoolers, and 64 infants, which includes up to 24 toddlers. Stock Farm Road Children's Center Preschool Component will have 5 approved classrooms (Red Hen, Five Little Ducks, Peter Rabbit, Sheep in a Jeep, and Chicken Little) . Facility will also have 2 play yards available for the preschoolers. The preschool rooms are equipped with varied age appropriate materials and equipment. There are 10 toilets, and 15 sinks available for children use. LPA reviewed staff records on 9/16/2021 at 2:24PM indicates that all staff members present have the appropriate criminal record clearance and or exemption. LPA reviewed (13) Personnel files were reviewed. (10) Children's files were reviewed. The classrooms and play yard are age appropriate and in good repair. Bathrooms are clean and in working order. The staff have a separate bathroom located within the facility. The sign in and out logs were reviewed and checked with current census. Outdoor play areas are free of hazards and provided a shaded area for the children and access to drinking water. There is a working telephone. Working carbon monoxide detector, smoke detector and fully charged fire extinguisher were observed at the facility. Opening and closing staff have current CPR and first aid training. LPA observed there are tight fitting lids on trash cans located inside the center. The center documents and conducts fire/disaster drills with the last one documented on 9/15/2021 . Facility provides AM/PM snacks with children bringing lunch from home. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Director is reminded that unusual Incidents must be reported within 24 hours by phone. Written report is to follow within 7 days utilizing form LIC624. LPA informed the Director that all forms can be downloaded at www.ccld.ca.gov and encouraged the Director to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The Director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com . This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children's, personnel, and administrative records. No citations issued during today's visit. Effective today Preschool Component will decrease from 92 preschoolers to 76 preschoolers. The Director was provided a copy of their appeal rights and the signature on this form acknowledges receipt of these rights. A Notice of Site visit was posted at the time of inspection and must remain posted for 30 days.
