Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 12:43 am and was greeted and met with the Licensee, YuJuan Chen. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), Parents' Rights (PUB 393) and Earthquake Preparedness Checklist (LIC9148). Facility's operating days and hours are Monday to Friday 7:00 AM to 5:30 PM. The last fire drill was conducted on 02/10/2025. Present are licensee, licensee's husbands, 2 helpers and 12 children, including 4 under 2 years old. LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. Licensee can't locate children roster. Licensee stated will find it and submit to LPA. There are 2 children files missing immunization records. Licensee stated will asked parents to submit immediately. LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee and her husband's pediatric 1st Aid and CPR certificate, expires January 2027. Licensee accompanied with LPA to tour the facility. Off limits areas inside the home are master bedroom and garage. Fire clearance report granted on 2/11/2019, noted the garage not allowed for child care use. Licensee stated bedroom 1 and 2 are on for child care. Licensee updated the LIC 999A. LPA observed the fire extinguisher in the kitchen, the smoke and carbon monoxide detector are in dining room and hall way. LPA provided a copy of Healthy Beverages in Child Care flyer(AB2084) to the licensee. The bathroom is clean and organized. LPA observed detergent and cleaning compounds are stored at under sink locked cabinet which are inaccessible to children. Continue on page 2 There are age-appropriate toys in the living room and play yard for children. Licensee stated there is no firearm in the facility. There is no stairs in the facility. There is no body of water observed outside of play yard. The outdoor play area is enclosed with appropriate fence and safe for children. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee stated there is no medication provided in the facility. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Continue on page 3 The Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. The licensee also confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Two type B citations were issued during today\342\200\231s inspection and a copy of appeal right was provided to the licensee. A Notice of Site Visit was given and must remain posted for 30 days.
