What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Collins, conducted an announced case management licensing inspection to the facility today, due to the facility request for a change of capacity. The facility is closed due to summer break, No children or staff in census today. LPA scheduled an inspection with Rachel Ratliff the site director. the preschool program is located in the same building of the Foothills Church. LPA toured and measured Room # 2 which is indoor area to be added to the indoor activity space during today's inspection. Fire clearance was granted 6/30/2021. INDOOR MEASUREMENTS were taken 07/16/2021 Room # 2: (13.25 ft.) x (21.67 ft.) = 287.13 sq.ft. (33. ft.) x (13.33 ft..) - (3.23 ft encumbered space) = 439.66 sq. ft. = 723.79 sq ft. INDOOR MEASUREMENTS ARE AS FOLLOWS taken 7/27/2018 : Room # 3: 17.583\342\200\231 x 5.750\342\200\231 = 101.102\342\200\231 plus 29.833\342\200\231 x 16\342\200\231 = 477.328 minus encumbered space 3.541' Room #4: 22.250\342\200\231 x 29.833 = 663.784 minus encumbered space 3.541\342\200\231 TOTAL INDOOR SPACE: 1235.132 sq. ft. + 723.70 sq. ft. = 1,958.922 sq. ft. divided by 35 = 55 pre-school children. There are 6(90) sinks and 7(105) toilets available for pre-school children. The hot water temperature was 110 degrees F. in the children's sink. LPA observed dispenser soap and disposable towels in the bathrooms facility is COVID-19 compliant. Staff have separate bathroom, located throughout the building, that can be utilized by sick children if necessary. Sick children will be isolated in the front desk office and isolation equipment is provided. The pre-school rooms have appropriate lighting & ventilation. First aid supplies are stored in each classroom and are inaccessible to children. Medications will be stored in locked cabinets in classrooms inaccessible to children and in a refrigerator in a locked room. OUTDOOR MEASUREMENTS ARE AS FOLLOWS taken 7/27/2018: PRESCHOOL PLAYGROUND AREA: 116\342\200\231 x 25.416\342\200\231 = 2948.256\342\200\231 plus 6\342\200\231 x 37.333\342\200\231 = 223.998\342\200\231 plus 35.916\342\200\231 x 22.250\342\200\231 = 799.131 plus 66.749\342\200\231 x 12.334\342\200\231 = 823.282\342\200\231 plus 11.583\342\200\231 x 49.416\342\200\231 = 572.385 TOTAL OUTDOOR SPACE: 5367.052 sq. ft. divided by 75 = 71 preschool children. LPA observed that the preschool playground areas are surrounded by appropriate fencing. The playground equipment is age appropriate. LPA observed there is a separate fenced playground for school age children (which is not included in the outdoor space). There is a climbing structure in the preschool playground and wood chips as resilient materials. Shade is provided by tarps. Drinking water will be provided by water dispensers and disposable cups. There is adequate equipment, supplies, and toys for children outdoors. LPA did not observe any bodies of water during today's visit. A review of staff records on 07/15/2021 indicates that the current staff who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA remaindered the Director of the applicable $100 civil penalty per person per day, a minimum of $100.00 to a maximum of $3000.00 per person for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. LPA conducted an exit interview with the Site Director prior to the conclusion of today's visit and advised them that a license for 41 preschool children wil lbe approved Notification of Site visit was given and must be posted to 30 days. 1) A review of this report and approval from a Licensing Manager No regulatory violations issued during this visit.
