Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Case Management-Other inspection. LPA met with Director Rachel Ratliff and explained the reason for the inspection. During today's inspection, LPA observed that the outside area has concrete parking blocks. Two of the concrete parking blocks are broken and the metal bar is exposed. LPA observed that the end of the metal bar are sharp. Director stated that they have requested that the concrete parking blocks be taken out. Director stated that they will barricade until it is removed. At 10:35AM, LPA observed that there were two children in the playground. There were two staff outside on the concrete area. There was one staff that was standing adjacent to the playground. A staff moved inside the playground during today's inspection. LPA discussed with Director about ensuring that children are supervised at all times, including visually. As a result of this inspection, a Type B and a technical violation were issued. Exit interview conducted and report was discussed and provided to Director Rachel Ratliff. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
