LPAs observed that children entered the building without any staff present and children went the restroom. LPAs also observed children walking from the restroom in the front building to the Pre-K without any staff present. Staff walked away from the bathroom.

LPA observed that the screen barricading the wall heater in the 2 years old room was broken; therefore, making the wall heater accessible to children. LPA also observed that the plastic picnic table was cracked.
