What Happened

Case management inspection revealed unlicensed use of second floor for isolation (no fire clearance), forced infant sleep practices violating regulations, missing infant sleep plan, and licensee providing false statements about child safety practices.

Observations

  • Second floor used as isolation area for sick children but fire clearance only granted for first floor areas (living room, family room, bathroom) as of 11/24/2020
  • Licensee forcing infant to nap/stay in designated sleeping area during 1-3pm nap time, violating infant safe sleep regulations
  • Licensee provided false statements to investigator denying children climbed out of highchair when text messages prove child C1 (over 1 year old) climbed out repeatedly
  • Licensee placed child C1 over 1 year old in highchair (only approved for children under 1 year)
  • Child C3 missing Individual Infant Sleep Plan (LIC 9227)
  • Three Type A violations and one Type B violation cited
