What Happened

Complaint investigation into failure to provide licensing reports to parents for Type A citation issued 10/03/2024. Licensee self-admitted to not maintaining signed LIC 9224 acknowledgement forms in children's files. Allegation substantiated.

Observations

  • Licensee failed to maintain signed LIC 9224 (Acknowledgement of Receipt of Licensing Report) forms in children's files for parents of enrolled children as of 10/03/2024
  • Licensee stated forms were provided to parents but was unaware of requirement to keep them in each child's file
  • Licensee acknowledged will now collect signed LIC 9224 forms from all enrolled children and any child enrolling within 12 months of the Type A citation date
