What the inspector wrote

As a result of this investigation, a Type A citation was issued. A civil penalty of $500 was assessed for immediate $500. Exit interview conducted and report was reviewed with Director Manali. A notice of site visit has been issued and must remain posted for 30 days. LPAs Ashley Lopez and Samantha Yip informed Director, Manali Doshi, that this report dated 03/29/2023 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPAs Ashley and Samantha informed the Director, Manali, to provide a copy of this licensing report dated 03/29/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

This requirement is not met as evidence by: Based on interviews, there was an incident where a child was left unsupervised outside.     REPORTING REQUIREMENTS

    Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Any unusual incident or child absence that threatens the physical or emotional health or safety of any child.This requirement is not met as evidenced by: Based on interviews and record reviews, there was an incident where a child was left unsupervised outside. Incident was not reported to Licensing, which poses a potential health and safety risk to children in care.
