Building and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement is not met as evident by: Based on observation and record reviews, facility had a wooden beam that went across the pergola that was about 3 feet off the ground. LPA observed that children had to bend down or squat down to go under. There was an incident where a child bumped into it and obtained a red mark on their face, which poses a potential health and safety risk to children in care.

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...This requirement was not met as evident by: Based on interview, Site Director Jessica stated that she did not report one of the cases of COVID to Licensing within the next working day. This poses a potential risk to the health and safety to the children in care.
