What the inspector wrote

Licensing Program Analysts (LPAs) Marilou Monico and Ashley Lopez met with Site Director, Svitlana Butenko, for the purpose of conducting a Case Management Inspection. The facility submitted an application to change the capacity from 53 to 56 children and removing the toddler option component on the license. Fire clearance was approved on June 6, 2023 . The Preschool Program operates Monday through Friday from 8:00 AM to 6:00 PM, serving children ages 2 years old to 6 years old. LPAs toured the facility during today's inspection as well measured the space available for the children. During the inspection, Site Director stated that they will use the outside room located in between Playground 1 and Playground 2 as the Office and use the prior "Office" as a classroom "Learning Room" for the children. Site Director submitted an updated facility sketch. The following are indoor measurements and information: Classroom 1: 38.750 x 23.000 = 891.25 minus 25.992 (encumbered space) = 865.258 Classroom 2: 39.833 x 13.917 = 554.355 minus 61.334 (encumbered space) = 493.021 Classroom 3: 39.833 x 13.917 = 554.355 minus 52.857 (encumbered space) = 501.498 Learning Room (previously the Office): 10.333 x 6.583 = 68.022 TOTAL PRESCHOOL INDOOR SPACE = 1,927.799 sq. ft. divided by 35 = 55 children The facility has 5 toilets (75) and 6 sinks (90) available for the children. The sinks used by children only have cold running water. There is a staff bathroom located next to the Learning Room. Sick children will use the staff bathroom if needed. The isolation area for sick children is located adjacent to the staff bathroom and isolation equipment will be provided. Staff and children's bathrooms are clean, sanitary, and operable. Continuation on Pages 2 & 3 LPAs observed containers with tight-fitting lids for the disposal of solid wastes. First aid supplies are stored in each classroom inaccessible to children. Cleaning supplies are stored in the laundry room and in the kitchen out of reach of children. LPAs observed smoke and carbon monoxide detectors, fire extinguishers, and appropriate lighting in each classroom. The facility has heating and air conditioning. LPAs observed that all furniture and equipment indoors are in good condition and safe for the children. Drinking water is readily available for the children indoors via water dispensers and disposable cups. There are 16 tables, 65 chairs, 88 cubbies, 40 hooks, and 62 cots for the children. There is adequate equipment, supplies, and toys for the children in each classroom. The following are outdoor measurements and information: Playground 1: 47.667 x 64.667 = 3,082.481 minus 172.516 (encumbered space) = 2,909.965 Playground 2: 20.333 x 63.750 = 1,296.228 TOTAL PRESCHOOL OUTDOOR SPACE: 4,206.193 sq. ft. divided by 75 sq. ft. = 56 children LPAs observed that the playgrounds are surrounded by appropriate fencing and the outdoor surfaces are safe. Shade is provided by trees, canopies, patio, and building overhang. Drinking water outdoor is provided via water dispenser and disposable cups. No bodies of water were observed. Site Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Continuation on Page 3: Exit interview conducted and report was reviewed with Svitlana Butenko, Site Director . LPAs advised Svitlana that a license for 55 preschool children will be granted pending the receipt of the following items: 1) Updated fire clearance for 55 preschool children and adding the Learning Room (previously the Office) on the license. 2) Photos showing that the Learning Room and Isolation Room are fully set up. 3) Revised Application for a Child Care Center License (LIC 200A) with a capacity of 55 children. 4) Upon receipt of the Plan of Corrections noted on Facility Evaluation Report (LIC 809) - Required 1 Year Inspection dated 06/26/23. 5) Approval from Community Care Licensing Management. As a result of this inspection, deficiencies are cited on the LIC 809- Required 1 Year Inspection dated 06/26/23. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
