What the inspector wrote

Type B deficiency was cited today as a result of the complaint investigation and is noted on the attached page LIC 9099-D. Exit interview was conducted and report was reviewed with Site Director/ Owner Svetlana Umansky. Appeal Rights were given. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

Director/Owner Svetlana Umansky was away from the facility from 09/18/2025-10/15/2025. S(1) was not present on 09/24/25-09/26/25, 09/29/25,09/30/25 and 10/13/25. S(2) acting director worked from 8:45AM, 8:45AM-2:45PM on 09/24/25-09/26/25, 09/29/25,09/30/25 and from 8:45AM-2PM on 10/13/25. Leaving the facility without a director during the reamining hours of operation.
