What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Collins conducted a scheduled Tele-Pre-Licensing inspection with Lina Arab Pre-K Director and Mona Nezzar Principal Licensee is requesting a change of location previous preschool site (1095 Dunford way BLD # 400 Sunnyvale Ca 94087), a change of capacity (60 preschool children), and name change ( Silicon Valley Academy Project). The preschool is located at 195 Leota Ave Sunnyvale CA. 94086 on the church campus of St. Cyprian. The campus also has an active private elementary middle school (Silicon Valley Academy Project) A Fire Clearance has been granted on 06/24/2021. The Preschool Center will operate Monday through Friday from 08:00AM to 05:30PM, serving children ages 2 years to 6 years old . Facility's phone # (408) 243-9333. The building was inspected inside and out. Classrooms 1, 2, 3 and Outdoor play space as well were measured to determine the maximum capacity that meets the requirement of 35 square feet of indoor activity space and 75 square feet of outdoor activity space per child. Preschool Indoor Activity Space: Preschool Room #1 = (Total sq. ft. of Classroom 1) \342\200\223 (Total Encumbered Space in Classroom A) = (870.17 sq. ft.) \342\200\223 (43.29 sq. ft.) = 826.88 sq. ft. Preschool Room #2 = (Total sq. ft. of Classroom 2) \342\200\223 (Total Encumbered Space in Classroom B) = (939.65 sq. ft.) \342\200\223 (46.45 sq. ft.) = 893.20 sq. ft. Pp2 Preschool Room #3 = (Total sq. ft. of Classroom 2) \342\200\223 (Total Encumbered Space in Classroom 3) = (931.73 sq. ft.) \342\200\223 (46.45 sq. ft.) = 885.28 sq. ft. Total Indoor Activity Space = (2,605.36 sq. ft.) divided by (35 sq. ft. / child) = 74. Children Preschool Outdoor Activity Space: Play Yard B = (Total sq. ft. of Play Yard B) \342\200\223 (Total Encumbered Space in Play Yard B) = (44,630.35 sq. ft.) \342\200\223 (658.348 sq. ft.) = 43,972.00 sq. ft. Total Preschool Outdoor Activity Space = (43,972.00 sq. ft.) divided by (75 sq. ft. / child) = 586.00 Children There are 11/165 toilets and 9 /135 for children to use. In addition, there is a restroom with one toilet and one sink across from near the director\342\200\231s office used for staff and ill children. Children who become ill will use this restroom and will be isolated in the office with isolation equipment. LPA observed soap dispenser and disposable paper towels in the restrooms. The sinks used by children have hot and cold running water. Staff will ensure temperature control valves under the sinks, are set to deliver water at temperature no higher than 105 degrees F. Pp3 There are cubbies and hooks for children to store their belongings. Classrooms are furnished with age appropriate tables and chairs, play equipment, and toys. The facility has cots for children to use for napping. Facility will provide nutritional morning snacks; parents are expected to provide lunch daily. LPA observed a refrigerator were items can be kept if necessary . There are trash cans with tight fitted lids, and paper towels and hand soap near the sink. Facility staff will clean toys and equipment daily with cleaning wipes and solutions that will be kept inaccessible to children. The facility is Covid19 compliant . Detergents, cleaning compounds, sharps, medicines, and other items which could pose a danger if readily available to children are stored where they are inaccessible to children in care. Drinking water will be readily available to children both indoor and outdoor. LPA observed that there is a fence that enclosed the Playground . The outdoor activity space is equipped with a variety of age-appropriate equipment such as two play structures. Areas around and under climbing equipment have artificial grass used as material that absorbs fall. Shade is provided by building overhang and gazebo. Director Lina Arab has obtained a Clearance for Criminal Background and Child Abuse Index Checks, She also meets the education and experience qualification requirement and had completed 16 hours of training on preventive health practices with Lead Poising , which included pediatric cardiopulmonary resuscitation and pediatric first aid, and one hour on Child Care Nutrition. Her Pediatric CPR/1st Aid Certification expires 11/2021. Director has proof of immunity against Measles and Pertussis, and Health Screening with Tuberculosis clearance. Her Mandated Reporter Training for Child Care Provider Certification expires 7/2023. Pp4 LPA reminded the Director and principal of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person. The facility plans to provide Incidental Medical Services \342\200\223 IMS. A Plan of Operation that includes IMS has been submitted to the Department. Facility is encouraged to frequent the Department\342\200\231s website at www.cdss.ca.gov [Shortcut: www.ccld.ca.gov ] to access resources for Providers, Regulations, Annual License fee, Adoption of new Laws, etc. Exit Interview was conducted, where this report was reviewed with Director and Principal Applicant was informed that once Licensing Office receives all required documents and waiver request the application for a Preschool Center License will be submitted for the final stage of review by Licensing Management.
