On 04/14/26, Licensing Program Analyst (LPA), Syeda Bahar conducted an unannounced case management visit at the facility, regarding an incident that occurred on 04/10/26. The incident was reported to the Department on 04/10/26. Upon arrival, LPA met with the Director, Ivania Rodas and explained the purpose of today's inspection. LPA toured the indoor and outdoor areas of the facility with the Director, Ivania Rodas and a census was taken. It was reported that a parent informed the facility of their concerns regarding classroom practices by one staff (S1). The parents reported inappropriate communication from staff (S1). The parent also expressed concern that one child (C1) had been removed from activities inappropriately by Staff (S1). The Director stated that upper management was notified, and a staff meeting was conducted to discuss the concern. During today’s inspection, LPA interviewed staff, reviewed records, and obtained copies of the facility roster, personnel report, and other supporting documents. Due to insufficient information available at this time, the reported incident requires further investigation. No deficiencies were cited during today's inspection. Appeal rights were provided. Exit interview conducted and the report was reviewed with the Director, Ivania Rodas. A Notice of Site Visit was given and must remain posted for 30 days.
