What the inspector wrote

On 05/19/26, Licensing Program Analyst (LPA) Syeda Bahar conducted an unannounced case management follow-up inspection regarding an incident that occurred on 04/10/26 and was reported to the Department on 04/10/26. LPA met with Director Ivania Rodas and explained the purpose of the visit today. The initial case management inspection was conducted on 04/15/26. It was reported that Staff (S1) used communication and classroom practices that did not support children’s personal rights. Per the incident report, Staff (S1) used inappropriate communication and removed one child (C1) from activities inappropriately. The facility was notified of the parents’ concerns on 04/10/26. During the initial case management inspection on 04/15/26, the LPA obtained and reviewed information regarding the incident involving Staff (S1) and Child (C1). During today’s visit, the LPA interviewed staff and reviewed documentation of the facility’s corrective actions. Based on interviews and records reviewed, it was determined that the facility has taken appropriate corrective measures. Staff member (S1) has completed disciplinary training, positive guidance training, and training in appropriate communication. The Director has increased supervision and monitoring to ensure compliance and to prevent this from happening again. LPA informed the Director about the Community Care Licensing Division’s Technical Support Program. The Director was advised to provide additional training to Staff (S1) on children’s personal rights. No deficiencies were cited during today’s inspection. Appeal rights were provided. One Advisory Note was issued during today’s inspection. Please see the attached Advisory Note page for information on the technical violation addressed today. Note that, technical violations are not citations but serve as advisory notes. Exit interview was conducted, and the report was reviewed with the Director, Ivania Rodas. A Notice of Site Visit was given and must remain posted for 30 days.
