What the inspector wrote

Licensing Program Analysts (LPAs) Samantha Yip and Ashley Lopez conducted an announced pre-licensing inspection. LPA met with Applicant Kamal Gill and Michael and explained the reason for the inspection. Applicant Rohan Gill arrived shortly after. The purpose of inspection is Applicants are applying for a preschool license for a capacity of 40 children. Applicant is also applying for a infant license at the same location. A fire clearance was granted on 03/07/2023. The measurement for the indoor and outdoor were conducted during inspection. The measurement are as followed: INDOOR MEASUREMENTS: Room 2 : (17.083 x 11= 187.913) + (10.167 x 12.750 = 129.629) = 317.542 Room 3 : (42.500 x 32.917= 1398.972) minus encumbered space (13.667 x 2 = 27.334) + (6.500 x 2.083 = 13.539) + [4(4.833x1)= 19.332] + [2(2.500 x 2) = 10] +(17.417 x 4.917 = 85.639) +( 2.083 x 8 = 16.664) + (2 x 2.500= 5) = 177.508 = 1398.972- 177.508 = 1221.464 Total Indoor Square Footage : 317.542 + 1221.464= 1539.006 divided by 35 = 43 children OUTDOOR MEASUREMENTS: (88.917 X 34.917 = 3104.714) +( 2.083 X 9.833 = 20.482) + (16.083 X 2.500 = 40.207) = 3165.403 minus encumbered space (24.750 x 3.500 = 86.625) + (7.585 x 8.167 = 61.930) = 148.555 = 3016.848 Total Outdoor Square Footage: 3016.848 divided by 75 = 40 children ------------------continues on 809 dated 03/13/2023 page 2---------------------- -----------------continuation of 809 dated 03/13/2023 page 1--------------------- There are 76 chairs, 12 tables, 55 cubbies, 4 toilets, 6 sinks, and 60 cots. There are toys for children. Facility will be providing meals and snacks to children. There is kitchen to prepare and store meals and snacks. Kitchen has running hot and cold water. Drinking water is provided through Brita filtered water. The outdoor area is fenced. Area around play structure has resilient material. LPA discussed with Applicant about ensuring that there are no holes in the resilient material. LPA also observed that the bolts in the toy train structure were missing bolts causing the bottom inner base board to come loose. It left a gap between the structure and the baseboard, which could pose a risk to children. Applicant stated that they will install bolt and send proof to licensing. LPA also discussed with applicant about power washing structure due to there being cobweb. LPA observed that there was moss under the slides, which was slippy. Shaded rest area is provided through canopy. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. -----------------continues on 809 dated 03/13/2023 page 3--------------------- ----------------continuation of 809 dated 03/13/2023 page 2----------------------- LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Exit interview conducted and report was reviewed with Applicant Rohan Gill. LPAs advised applicant upon approval of Licensing Management, a license for an preschool center license will be granted and issued to Applicant and upon completion of the following: - baseboard of the train structure is bolted - moss under the slide is removed
