Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced Case Management-Other inspection. LPA met with Director Michael Frugoli and explained the reason for the inspection. The purpose of this inspection is LPA observed that there is a half wall installed that divides the room in half. The half wall goes from the wall that is perpendicular to the kitchen to the wall to the left of the door that leads to the preschool yard. There are doors at each end of the wall. LPA interviewed staff. LPA interviewed Licensee Kamal Gill on the phone. Licensee stated that the wall was installed after the pre-licensing inspection. LPA discussed that the Licensee needs to inform Licensing if they are going to make any alternations to the building and that a fire clearance is needed. LPA obtained an updated LIC 999 to reflect the change to the facility. As a result of this inspection, a Type A citation was issued. Exit interview conducted and report was reviewed with Licensee Kamal Gill. A notice of site visit has been issued and must remain posted for 30 days. LPA Samantha Yip informed Licensee Kamal Gill that this report dated 06/30/2023 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Samantha informed the Licensee, Kamal, to provide a copy of this licensing report dated 06/30/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
