On 6/4/24, at 3:28 PM, Licensing Program Analyst (LPA) Liridon Fici arrived unannounced to conduct a Case Management Visit. LPA met and was greeted by Licensees, Kamaljit and Sukh Gill and explained the purpose of today\342\200\231s visit. During today's visit, Licensee informed LPA there will be a new Director for this center, however, Licensee did not notify CCL within 10 days after their old director left on 4/26/2024. Licensee submitted a directors packet for LPAs review. Licensee informed LPA on 5/27/2024, there was water damage in the infant's room and the center closed their doors on 5/27/2024, and 5/28/2024 until further notice. LPA asked Licensee if there was an incident report that was submitted to CCL, and Licensee stated there was no incident report was submitted to CCL. The following type B deficiency was cited on the attached page (809-D). Licensee was informed that failure to correct the deficiency by the specified Plan of Correction (POC) Due Date may result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Licensee, and a copy of this report reviewed and provided along with appeal rights.
