Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA), Marilou Monico, conducted a Case Management Inspection. LPA met with Licensee, Nikitha Kandukuri. LPA and Licensee toured the facility. At approximately 8:55 AM, LPA observed three staff members supervising 3 infants and 5 preschool age in the Breakfast Room. Licensee indicated that a waiver request was submitted to Licensing to commingle the three programs (infant, preschool, and school age) in the morning from 6:30 AM to 7:30 AM in the Breakfast Room and in the afternoon for the infant and preschool from 5 PM to 6 PM in the Panda's Room. LPA advised licensee that the three programs shall be physically separated and can only commingle once the waiver request is approved by Licensing. As a result, Type B deficiency is cited on the following page. Exit interview conducted and the report was reviewed with Licensee, Nikitha Kandukuri. A Notice of Site Visit was issued and must remain posted for 30 days.

This poses a potential health, safety and personal rights risk to children in care.
