Licensing Program Analyst (LPA), Liridon Fici-Doni, met with Nikitha Kandukuri, Licensee/Owner for a 1 Year Inspection and informed her the purpose of visit. LPA toured both indoors and outdoors during the inspection with Licensee. LPA observed two (2) infants with one (1) Teacher in the Baby Guppies class, 2 children with 1 Teacher and Aide in the Busy Bee's class, 12 Children with 1 Teacher and Aide in the Playful Pandas class, and Six (6) Children with 1 Teacher in the Terrific Tigers class. The center was in compliant with the facility license capacity and ratio requirements. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Child Car Seat Law (PUB 269), Menus, and Activity Schedule. Days and hours of operation are Monday through Friday 6:00AM to 6:00PM. Licensee stated the center does provide transportation to School-age children. LPA reviewed sign-in/out sheets, the facility roster (LIC9040), and the fire/disaster drill log during today’s inspection. The last fire/disaster drill was conducted on 4/14/2025 , which is compliant with the six-month requirement for facilities. LPA observed a fully charged 2A10BC fire extinguisher (last serviced: 5/13/2025 ), with functioning smoke detector and carbon monoxide detector. Licensee stated that she does not currently have Incidental Medical Services on file and does not administer medication at this time. There are no weapons or firearms on the premises. LPA did not observe any bodies of water during inspection. Page 1 of 4. Indoor areas of the facility were inspected by the LPA today and observed to be clean, orderly, and safe for day care children. The infants, Pre-schoolers, and School-Age rooms are physically separate from other childcare center components at the facility. LPA observed sufficient age-appropriate materials, toys, and play equipment for infants, preschoolers and School-Age children. Toys are safe and do not have sharp edges or small parts that may pose a choking hazard. Children's changing table was observed to be padded, within arm’s reach of a sink, in good repair and safe condition. Cribs are used by infants, which are free from loose articles, covered with a fitted sheet, and there are no objects hanging above or attached to them. The floors are clean and free of tripping hazards and waste containers have tight fitting lids. LPA reviewed five (5) staff files which contained the required forms/documents, including current CPR and First Aid certifications on file. All staff members had current Mandated Reporter Training on File. Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during any off-site activities. LPA reviewed five (5) children's file, which contains all required forms/documents, including Admission Agreement, Emergency Medical Consent (LIC 627), and Information and Emergency Information form (LIC 700). LPA reminded licensee that infant Needs and Services plan shall be updated at least quarterly and shall be completed with the infants authorized representative and a representative from the facility, such as a Licensee or director. The feeding plan is current for all infants and licensee understands that all formula and bottles provided should be labeled individually with the child’s name and date. Licensee understands the conditions, limitations, and capacity specifications of the Facility license. She understands that children shall be always visually supervised. Any child(ren) who exhibit symptoms of illness including, but not limited to, fever or vomiting, are not accepted into care. Any child(ren) who become ill during the day, shall be isolated in the director’s office area. LPA observed that the Facility is clean, safe, sanitary, and in good repair for children, staff, and visitors. Licensee states the center does not have a Janitorial Cleaning Service, however, staff will clean the center at the end of the day once all children are gone. Page 2 of 4 Licensee understands that the Facility must be kept free of flies and other insects & rodents. LPA observed that furniture and equipment is in good condition and safe for the children. Children have their own water bottles labeled with their name on it, when playing outdoors there are water stations for the children along with, they have their water bottles. LPA observed there is cold running water, refrigerator, and microwave on the premises. Breakfast, AM snack, lunch and PM snacks are provided by the facility. The Facility has trash cans with tight fitting lids for solid waste in the classroom. Cleaning supplies are inaccessible to the children and stored in cabinets located in areas off limits to children. LPA observed a complete first aid kit available in the facility. The playground area utilized by children is surrounded by appropriate fencing and the outdoor surfaces are safe for the children. LPA observed that the outdoor equipment is age appropriate and in good condition. Shade rest areas are provided by a pergola. There are sufficient resilient materials on the outdoor playground area. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility does not provide Incidental Medical Services (IMS) at this time. Incidental Medical Services (IMS) policy was discussed. LPA did not observe an IMS plan on file, and LPA gave Licensee a copy of the IMS guidelines for her reference. Page 3 of 4. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1-CCP ). Licensee understands that children's personal rights should not be violated, including no unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature. LPA provided Licensee with website resources on managing food allergies at school and handling medical emergencies related to food allergies.
