What the inspector wrote

Licensing Program Analyst (LPA) Marilou Monico met with Site Director, Suhasini Ramakrishnan, for an unannounced Case Management Inspection. On 10/17/24, police officers were dispatched to the facility to investigate an alleged injury to a child (C1). This is considered an unusual incident and the facility failed to report to Licensing. As a result, Type B deficiency is cited on the following page. Exit interview conducted and the report was reviewed with Site Director, Suhasini Ramakrishnan. Copy of appeal rights was provided to Site Director. A Notice of Site Visit was issued and must remain posted for 30 days.
