What the inspector wrote

The investigation revealed that child 1 (C1) was observed to have a burn mark on C1’s arm at pick up. C1’s parent brought the injury to the director’s (S1) attention, and S1 assisted in providing first aid to C1. No incident reports were reported to licensing within 7 days. Based on interviews, record review, and evidence gathered during the investigation process, the Department concludes that “The above allegation is thus found to be SUBSTANTIATED , meaning the allegation is valid because the preponderance of the evidence standard has been met. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with the Director, and this report was reviewed and provided along with appeal rights. Based on interviews and evidence gathered during the course of the investigation, it is concluded that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur; therefore, this allegation is UNSUBSTANTIATED . A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with the Director, and this report was reviewed and provided along with appeal rights.
