On 05/21/2026, Licensing Program Analyst (LPA) Deanna Villagrana met with Office Admin, Sarah Holloway for an unannounced case management visit. LPA explained the nature of the visit. LPA observed 10 infants in room 2 with one teacher and two aides, nine preschool children in room 7 with one teacher and one aide, 12 preschool children and two teachers on the preschool playground and six preschool children with one teacher and another staff in room 10. During LPA's walk through, LPA observed staff 1 was not associated to the facility. LPA contacted SJRO and obtained an updated roster. LPA observed staff 1 was associated as of today 05/21/2026 to the facility. Sarah Holloway stated she believes staff 1 began coming to the facility back in January after the holidays and comes several times a month and teaches the children technology. She stated staff 1 is employed by Action Day Schools and associated to other locations. No deficiency was cited. A notice of site visit was given to Sarah Holloway and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal rights were provided.
