What the inspector wrote

Throughout the investigation process, it was found the allegation is SUBSTANTIATED; based on interviews, observations, and information gathered by LPA. A finding that is SUBSTANTIATED means the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. As a result, one type B deficiency was cited on the following page and copy of Appeal Rights was given. Exit interview conducted in Mandarin with Licensee, Yaping Bai. A notice of site visit was issued and must remain posted for 30 days.

(b) No more than three infants are cared for during any time when more than 12 children are being cared for.This requirement was not met as evidenced by:Based on LPA observations, reocrds review and interviews, that licensee stated she had 13 children (including 4 infants) in April. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
