What the inspector wrote

Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Complaint Investigation and Annual Inspection and was met with the Licensee, YaPing Bai. LPA observed the required posted materials by the entry door, including the Facility License, Emergency Disaster Plan (LIC610), Parents' Rights (PUB 393) and Earthquake Preparedness Checklist (LIC9148). Facility's operating days and hours are Monday to Friday 8:30 AM to 6:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 06/06/2025. Present are licensee, 1 helper and 12 children, including 3 under 2 years old. The 3 infants 15 minutes check sheets are current and up-to-date. LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate, expires 02/25/2026. Licensee accompanied with LPA to tour the facility. Off limits areas inside the home are kitchen, garage and all rooms on the 2nd floor. The kitchen and stairs to the 2nd floor are barricaded with gates which are inaccessible to children. LPA observed the fire extinguisher in the bathroom, the smoke and carbon monoxide detector are in family room and hall way. The bathroom is clean and organized. There are age-appropriate toys in the living room and play yard for children. Licensee stated there is no firearm in the facility. There is no body of water observed outside of play yard. The outdoor play area is enclosed with appropriate fence and safe for children. Licensee has a cat stays in garage converted room which is off limits area. Continue on page 2 Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee stated there is no medication provided in the facility. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA encouraged the Licensee to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates. The Licensee can also email at childcareadvocatesprogram@dss.ca.gov and ask to be added to the email list for the updates. The Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Exit interview conducted and report was reviewed in Mandarin with the licensee, YaPing Bai. The licensee also confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. No citation was issued under today's annual inspection. 1 type B citation was issued under today\342\200\231s complaint inspection report (LIC 9099-D) and a copy of appeal right was provided to the licensee. A Notice of Site Visit was given and must remain posted for 30 days.

The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
