Inspector’s narrative

What the inspector wrote

On 12/09/2025 at 10:00am, Licensing Program Analysts (LPAs) Christina Uribe and Diana Campos conducted an unannounced visit. LPAs met with licensee, Olga Diaz Martinez. Also present at the time of today\342\200\231s inspection are 1 fingerprint cleared staff, licensee's adult step-son, and 5 children. During today's visit, licensee's step-son was observed to be present in the home. Per licensee, the adult step-son turned 18 years old in September 2025. Since the adult step-son has turned 18, he has not completed the criminal background check which is required for all adults residing in the home. According to the licensee, the adult step-son turned 18 while he was out of the state and returned recently on 11/30/25. This is resulting in the issuance of a Type B Violation for Criminal Record Clearance. Please refer to the attached LIC 809D page for additional information. Report was reviewed and the Notice of Site Visit was given and must be posted for 30 days. Appeal rights were given. Exit interview was conducted and report was reviewed with the licensee, Olga Diaz Martinez.
