What the inspector wrote

On Tuesday, February 18, 2025 at 11:30 AM, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Caroline Colson met with Applicant Yaqin Qui, Hong Zhang, Friend, Jeffrey Hogg, Husband, for a case management inspection to request 24 infants for an Infant Component with the ages 2 months - 24 months. The facility is licensed for 30 Toddlers for the current Toddler Component with ages 18 months - 36 months. Licensee requests a decrease in capacity for the Preschool Component to 30 preschool children from 2 years of age to first grade entry. The fire clearance was granted and received. The facility operates on Mondays through Fridays from 8:00 AM to 6:00 PM. INDOOR ACTIVITY SPACE: Classroom 1 is the classroom for the Preschool Component. The total measurements for the Preschool Component is 1051.09 square ft. which will accommodate the Applicant's request for 30 children. Classrooms 2 and 3 are for the Toddler Component. The total measurements for the Toddler Component is 1001.73 square ft and will not accommodate Applicant's request for 30 children. Classroom 4 is for the Infant Component. The total measurement for the Infant Component is 745.69 square ft which will not accommodate the Applicant's request for 24 children. There are six (6) sinks and six (6) toilets with one potty chair for the Infant Component. The staff bathroom will be the isolation bathroom and an area in each classroom will be designated for the isolation area. Please See LIC 809 C for Additional Information OUTDOOR ACTIVITY SPACE: The Preschool Outdoor Play Area is 2,257.28 square ft. and will accommodate the Applicant's request for 30 children. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. The following items are required before a license will be issued: 1. Waiver for Infant Outdoor Play Area and Toddler Outdoor Play Area approval 2. Parent Handbook, Employee Handbook, Admission Agreement, Facility Sketch will need to be updated. 3. Adequate Lighting needs to be available in all classrooms. 4. Applicant needs to update menu 5. Applicant will need to clean the carpet in Classroom 4 6. Applicant will need to ensure that the gate for the napping area is at least 4 ft high. 7. Applicant will need to purchase thermometers, tweezers and antiseptic solutions for all first aid kits. 8. Post all required documents for the Licensing Board. Exit interview conducted and report was reviewed with Applicant Yaqin Qui
