What the inspector wrote

The Licensee is in violation of Reporting Requirements Section 101212 (d) Upon the occurrence, during the operation of the child care center of any of the events….(B) Any injury to any child that requires medical treatment…. The preponderance of evidence standard has been met; therefore, the above allegation is to be SUBSTANTIATED. LPA Estoesta informed the Authorized Representative Selva Namokarean that this report dated 7/23/2025 included a Type B Citation which can be posted as there is a potential risk to the health, safety, or personal rights of children in care. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed Authorized Representative Selva Namokarean.

This requirement is not met as evidenced by, based on LPA’s interview, C1 had an “allergic reaction” on C1’s face from a “food” that C1 ate in the classroom during lunch. This happened sometimes in April to May 2025, the exact date was not confirmed. S1 administered and provided “Zyrtec” medication. There was no incident reports made to the Department. This poses a potential risk to the health, safety or personal rights to children in care.
