What the inspector wrote

On 01/15/2026 at 11:20am, Licensing Program Analyst (LPA) Jialing \342\200\234Julianne\342\200\235 Zhu conducted an unannounced case management inspection for the purpose of investigating a reported incident. LPA met with Director Yaling \342\200\234Eileen\342\200\235 Lin. Also present during the inspection were six (6) fingerprint-cleared staff and 24 children in care. The facility is within ratio and in compliance with capacity regulations today. The incident occurred on 01/07/2026 and was self-reported to Licensing on 01/08/2026. Per director, a child (C1) may have sustained an injury that was caused by a staff (S1). The injury did not require medical attention. Parents were notified the same day by telephone call. An accident report was also filled out and provided to parents at pick-up. When the incident was self-reported, Director shared camera footage with Licensing. Footage was also shared with LPA Zhu on 01/09/2026 via email. During today\342\200\231s inspection, LPA reviewed the above information with Director and conducted interview to obtain more information. LPA obtained written statement from S1\342\200\231s account of the incident. Director stated she reviewed the camera footage of the events leading up to the incident, and the footage supported S1\342\200\231s account. Director was unable to show LPA the footage during today\342\200\231s inspection as the footage is only readily available for 7 days. LPA also obtained a photo of the injury that was taken on the day of the incident. A copy of the accident report was also obtained. LPA reviewed the footage of the incident that was shared again. LPA briefly observed the classroom where the incident occurred. C1 was in the classroom. Based on the evidence that was available, LPA determined the injury was likely not caused by the staff. No deficiencies were cited during today's inspection. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Yaling \342\200\234Eileen\342\200\235 Lin. Page 1 of 1. End of Report.
