What the inspector wrote

At 8:55AM, Licensing Program Analyst (LPA) Angela Luz met with Licensee Adina Jackson to conduct an unannounced annual RANDOM inspection. Present for this inspection was 5 children (2 infants and 3 preschool aged children), 1 fingerprint cleared assistant, and 1 fingerprint cleared adult in the home. The home was toured to conduct a Health and Safety Inspection. Days and hours of operation are Monday \342\200\223 Friday 7:30AM-5:30PM. Facility: The ON LIMIT AREAS are living room, nap room, kitchen, backyard, and daycare bathroom. The OFF LIMIT AREAS are the entire second floor, garage, car shop, driveways, laundry room, and supply closet. Off limit areas are inaccessible by closed and/or locked doors and visual supervision. Staircase to the second floor has a child gate and the wall heater in the kitchen has a barricade to prevent access by children in care. The home is neat and clean with heating and ventilation for safety and comfort. The ISOLATION AREA is the nap room. The outdoor play area is free from defects or dangerous conditions and is fully fenced. There is a large play structure that is bolted to the ground and has a resilient material underneath to protect from falls. There have been no changes from the areas previously identified as OFF LIMITS or alterations to existing building or grounds. There are no bodies of water. All hazardous materials and toxins are kept out of the reach of children in high cabinets. Licensee states that there are no firearms in the home. The home has a fully charged 3A40BC fire extinguisher that was last serviced on 10/26/23, working smoke detector, working carbon monoxide detector, and working telephone. LPA reminded that fire extinguishers should be serviced once a year and Licensee made an appointment to get it serviced tonight. ***Page 1 of 4*** Care and Supervision : There are ample age-appropriate toys that appear to be safe and in good condition. Licensee uses talking with the child as a form of discipline. Licensee understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, interference with eating, intimidation, or other actions of a punitive nature. Children are treated with dignity, receive safe, healthful, and comfortable accommodations. LPA observed 1 play yard. Other children in care sleep on cots. The play yard does not hinder entrance or exit to or from the space where infants are sleeping. The warning label for the play yard states to only use the mattress/pad provided by manufacturer. The mattress is firm and covered with a fitted sheet that is appropriate, fits tightly, and overlaps the underside of the mattress. The bedding is used for them only and cleaned weekly or before use by another infant. There are no loose articles and objects, bumper guards or objects hanging above or attached to the side of the cribs or play yards. Licensee understands Infant Safe Sleep requirements. Records: The Licensee CPR and First Aid certificate is current and expires 8/14/25. Licensee Mandated Reporter Training for Child Care Providers is current and expires 5/16/25. Licensee was reminded Mandated Reporter Training (AB1207) and CPR/First aid certifications needs to be renewed every two years. A copy of the licensee\342\200\231s immunization is on file. The Licensee conducts and documents fire and disaster drills every six months, last drill was on 12/18/24. A current copy of the facility roster was viewed, and a copy was obtained. Licensee carries liability insurance. Entrance checklist for Family Child Care Home was provided and licensee was reminded of documents to be posted in a prominent, publicly accessible area of the facility. LPA reviewed eight (8) children files during today's inspection for the following records: Notification of Parents Rights (LIC995A), Consent for Emergency Medical Treatment (LIC627), Identification and Emergency Information (LIC700), and Immunization records. C6 and C7 were missing a signatures on one of their forms. LPA reminded Licensee to keep children\342\200\231s immunizations records updated. Documentation of infant sleep checks every 15 minute that notate any signs of distress were missing. LPA reviewed requirements for the infant sleep check documentation with Licensee. Licensee will start documenting infant sleep checks today. ***Page 2 of 4*** LPA reviewed two (2) staff (S1-S2) files for the following records: Employee rights (LIC 9052), Statement Acknowledging Requirement to Report Child Abuse (LIC 9108), immunization records, and required training. Staff 2 is missing immunization record on file at the home and mandated reporter training. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Please complete the survey and share your inspection experience. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/ safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. During the exit interview, the LICENSEE, Adina Jackson, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Megan's Law was checked on 3/26/25. During today's inspection, 2 Type B deficiencies are issued on attached page 809-D. S2 missing mandated reporter training. 15 minute infant sleep checks missing. 3 advisory notes are issued. Keep immunization records up to date for all enrolled children. Keep immunization records for staff on file at the home. Ensure that all required licensing forms for enrolled children are complete with signature and date. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Adina Jackson.
