What the inspector wrote

On April 1, 2026, Licensing Program Analyst (LPA) Darnella Barnes conducted an unannounced annual inspection at the facility. The purpose of the visit was explained to Site Supervisor Rosalba Torres, who received the LIC125 Entrance Checklist and granted access. During the inspection, Site Supervisor, Program Manager Maria Hurtado, two teachers, and 10 children were present. The facility operated Monday through Friday from 8:00 a.m. to 4:00 p.m. Meals for the facility were prepared at another Community Bridges EED location and were delivered to the facility, daily where they were kept in the kitchen and were served to the children, and the facility offered water, AM snack, PM snack, and lunch. LPA observed a working smoke detector and a carbon monoxide. Required postings, including the activity schedule, menu, were present. The most recent fire drill was conducted on 3/17/26. LPA Barnes informed the Site Supervisor of the requirement to conduct and document fire drills at least once every six months. LPA observed a 2A-10-BC fire extinguisher with a service date of 4/24/25, and an operating telephone was present. Site Supervisor stated that no firearms were stored or present on the premises and that smoking was not permitted. -----CONTINUED NEXT PAGES ----- LPA Barnes inspected the indoor environment. The flooring did not present tripping or slipping hazards. Toy containers had smooth edges, ventilation, and lids that prevented finger entrapment. Each child had an individually labeled storage space. Bathrooms and toilets met required ratio regulations and were cleaned daily. Cleaning supplies and chemicals were stored in locked cabinets separate from food and out of children’s reach. Classroom floors were swept, carpets were vacuumed, and floors were mopped daily. Each child had individual bedding that was not shared, and bedding cots were cleaned daily. Bedding for the cots and blankets were sent home weekly to be cleaned and returned. LPA Barnes reviewed five children’s files during the inspection and found them complete. Parents signed in and out using the HUBBE app. LPA Barnes reviewed the sign-in and sign-out records, recording the date and time LPA Barnes reviewed three staff files. All files were complete. LPA Barnes informed the Site Supervisor that Mandated Reporter Training was required every two years. LPA Barnes stated that at least one staff member was required to maintain current CPR and First Aid certification at all times. LPA Barnes stated that aides and assistants were required to remain under the supervision of a qualified teacher except during naptime or bathroom transitions During the outdoor inspection, LPA Barnes observed that the play yard and the corner portion of the yard contained dry, overgrown grass and weeds along all edges, resulting in a Citation B on the attached LIC809D. All walkways, ramps, and stairs were free of hazards. The outdoor space was fenced at least five feet high, was free of debris, and did not contain any accessible hazards or bodies of water. First aid supplies were observed in an accessible location for staff and out of children’s reach. A waiver was in place to associate all criminal record clearances for facility staff under one central administrative facility license, Community Bridges EED Sycamore Street Center (Lic# 444400415). -----CONTINUED NEXT PAGES ----- Site Supervisor was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with Interim Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the Interim Director shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . -----CONTINUED NEXT PAGE ----- The Site Supervisor stated that the water from the sinks was used only for washing hands and cleaning, not for drinking. Drinking water was provided to the children in water bottles purchased for them. LPA observed water bottles in each classroom. Site Supervisor was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California..
