Licensing Program Analyst (LPA) Jaime Gonzales was greeted and met with Staff, Shaneesha Herrera for an unannounced annual inspection. LPA was granted access to the facility and toured both indoors and outdoors during the inspection. Upon arrival, LPA completed a census head count in the facility. LPA observed 11 preschoolers in the classroom and two Staff. During the inspection, Site Director LaVeta Lipscomb arrived and took over for Staff, Shaneesha Herrera. LPA observed all required postings in the entryway of the facility. Hours of operation for the facility are Monday – Friday, 7:30AM-4:30PM. The facility has a wavier for the outdoor play yard capacity which shall never exceed 16 children at any one time. All conditions of the waiver are being met. LPA observed fully charged 2A10BC fire extinguisher, functioning smoke and carbon monoxide detectors. The isolation area for sick children is in the classroom that is next to the staff room. Facility conducts Fire/disaster drills at least every 6 months and it was last conducted 01/13/2026. LPA collected a facility roster (LIC9040). LPA did not observe a fireplace, pool, spas, or other types of bodies of water. LPA observed open faced heaters located in the classroom next to the kitchen and the classroom that is next door to the sanctuary. The open faced heaters are elevated and inaccessible to children. LPA observed barricaded stairs in the facility that lead up to an off limits storage area. There is a staff room that is off-limits to children located to the right of the stairs. Site Director stated that the facility does not transport children. Lunch is provided by authorized representatives and the facility provides morning and afternoon snack. The menu for snack is posted on a monthly basis. The facility has a kitchen area and there is a stove that is used for making snacks. Site Director stated that staff will use the oven when the children are outside. Continued on page 2... The classrooms have age-appropriate materials, toys, furnishing, equipment, & adequate storage for children’s belongings. LPA observed all furniture and napping equipment is in good condition and safe for the children. Bedding is provided by the authorized representatives or provided by the facility and is washed washed weekly or as needed. Drinking water is readily available for children by water bottles labeled with the children’s names. Water bottles and filled throughout the day by staff via sink in the kitchen that has filter. Chemicals are stored in an inaccessible closet that is located in the kitchen area. The outdoor play areas are fully fenced. Shade is provided by retractable awnings and sail shades. There are age-appropriate outdoor toys and materials for the children. LPA did not observe a large play structure in the outdoor play area. LPA observed a covered sandbox and a barricaded air conditioning unit in the outdoor play area. There is a side yard to the right side when facing the building that is off-limits to children. LPA reviewed 3 staff files for the following records: Health Screening Report (LIC 503), TB Test, Statement Acknowledging Requirement to Report Child Abuse (LIC 9108), Employee rights (LIC 9052), staff qualifications, Mandated Reporter Training. During record review Staff (S2 and S3) were missing mandated reporter training in their personnel files. During record review all staff was missing Pediatric First Aid/CPR training. Site Director was reminded of their responsibility to renew Pediatric CPR/First Aid and Mandated Reporter Training every two years. LPA reviewed 5 random children files during today's inspection for the following records: Admission Agreement, Personal Rights (LIC 613A), Identification and Emergency Information (LIC 700), Notification of Parents Rights (LIC 995A), Consent for Emergency Medical Treatment (LIC 627), Physician’s Report with TB test (LIC 701). Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA discussed the safe sleep regulations with Site Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed Site Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
