Licensing Program Analyst (LPA) Elizabeth Larios and Licensing Program Manager (LPM) Joel Segura conducted an unannouced inspection to deliver the complaint allegation listed above. LPA met with staff, Faith Filey and explained the purpose of the visit. Based on the available evidence, it is concluded that although the allegation listed on this complaint may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is therefore UNSUBSTANTIATED. No deficiency was cited. Exit interview was conducted, where this report was reviewed and discussed with Faith Filey. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
