Child Care Center Directors Qualifications and Duties - (d) The child care center director, or the substitute director as specified in (f) below, shall be on the premises during the hours the center is in operation. This requirement is not met as evidenced by:Based on interviews and records review, the center has no full time qualified director on site. This poses a potential risk to the health, safety and personal rights to children in care.

Isolation for Illness - A center shall be equipped to isolate and care for any child who becomes ill during the day. (4) In combination centers, only one isolation area that serves all licensed components is required. This requirement is not met as evidenced by: The center's designated isolation area for sick children is the office , however there were times the children were isolated in the classroom when they become ill. This poses a potential risk to the health, safety and personal rights to children in care.

Reporting Requirements - Each licensee or applicant shall furnish to the Department reports as required by the Department.This requirement is not met as evidenced by: The facility did not notify Licensing of the changes in their hours of operation. This poses a potential risk to the health, safety and personal rights to children in care.
