What the inspector wrote

Licensing Program Analyst (LPA), Goodell met with applicant Pam Cayton and Mary Thompson for an Change of Location Inspection. Fire clearance was obtained was obtained. The facility\342\200\231s hours of operation are Monday - Friday, 8:00am- 5:00pm. Applicant has requested a preschool license for 30 preschool children with the age range of 2 years to 6 years old. INDOOR ACTIVITY SPACE: The facility has 2 preschool classrooms named Rainbow and Mandala. LPA observed the classrooms are equipped with child size furnishings and bookshelves. LPA observed individual cubbies for storing children's personal belongings and napping equipment. There is no kitchen and applicant will submit proof of classroom fridge. Children will bring their own meals and snacks from home. The center will keep back up food at the center for emergencies. There is a first aid kit in each classroom. Medication will be administered and stored inaccessible to children. Water is available indoors through personal water bottles. LPA observed 3A10BC fire extinguisher both classrooms. LPA observed smoke detectors located in both classrooms. LPA observed one functioning carbon monoxide detector in Rainbow classroom . Applicant will submit proof of second carbon monoxide detector for second for Mandala classroom. The total indoor space measured a total of 886.74 square feet which does not accommodate the applicant's request for 30 preschool children. Therefore applicant will but an updated LIC200A requesting capacity of 25 preschool children. An updated fire clearance will also be requested. LPA observed 2 toilets and 1 sink located next to Mandala classroom with 2 sinks accessible to children in both classroom. LPA also observed a separate private restroom for the staff that will not be used by church members during facility hours of operation. Report continues on LIC809-C The outdoor play area is fenced and is equipped with age-appropriate equipment. LPA observed play structure is for ages 5 years and up. LPA observed climbing structure and slide with cushioning to absorb falls. Shade is provided via the building overhang. Water will be available to children by personal water bottles. The outdoor play area contained a total of 3155.96 square feet which will accommodate the applicant's updated request for 25 preschool children. LPAs discussed Incidental Medical Services \342\200\223 IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm LPA discussed the Immunization Regulations SB 792, the requirement that all individuals working or volunteering at a licensed Child Care facility must have vaccinations against, Pertussis, Measles and Influenza. LPA advised the director that they can sign a declaration to be exempt from the influenza vaccinations however; Pertussis and Measles are not exemptible. To be exempt from Pertussis and Measles staff must have a medical exemption signed by a licensed physician. LPA discussed the new Mandated Reporter Training. Beginning January 1, 2018, Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years . Volunteers are encouraged but not required to take the training. In addition, effective January 1, 2018: Applicants must meet requirements as a precondition to licensure, existing licensees must meet requirements by March 30, 2018, new employees shall have 90 days from date of employment to complete training as required. This training requirement may be met by using the Department\342\200\231s Office of Child Abuse Prevention (OCAP) online training modules. LPA also reviewed with the director the Notice of Incomplete Application LIC184 for the remaining documents to be obtained. LPAs encouraged the director to the visit the department's website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining child care centers. The following to be submitted prior to approval of licensure. 1. Required documents listed on LIC 184 2. Proof of Carbon Monoxide detector for Mandala classroom 3. Proof of classroom fridge 4. Proof of Parent Board with required licensing for 5. Proof of covered trash cans. 6. Proof of Fingerprint clearance and association for Pam Cayton. 7. Updated LIC200A requesting 25 preschool children ages 2 to 6 years old. 8. Updated Fire Clearance.
