What the inspector wrote

An unannounced case management inspection was conducted today at 8:46 am by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Danielle Tuttle. In response to an Unusual Incident Report received by the Department on 9/9/25. A child in care, C1 was playing on the playground and jumping off the platform when they fell and broke their arm. The Site Supervisor, Danielle Tuttle was interviewed on 9/23/25 at 8:58am and stated that on 9/5/25 at 9:05 am the site supervisor was informed C1 was jumping off the platform of the climbing structure and landed on their bottom and left wrist. The Site supervisor applied ice to the wrist and contacted C1's parents to come and assess the injury. C1 was brought home and it was believed that the injury could be a sprain. After injury was assessed it was determined that C1 needed to visit a doctor. Doctor determined C1 had broken their wrist. One staff and one child were interviewed on 9/23/25 and confirmed C1 jumped off the platform and landed on their wrist. C1 showed LPA how they could jump off the play structure. During today\342\200\231s inspection, the facility was toured and the play equipment was observed. Attendance files and staffing information was reviewed. Ouch report was reviewed. LPA observed children jumping and playing on the playground. LPA photographed the play equipment that was approved at the pre-licensing visit to be appropriate for preschool age children. Based on interviews and record review it was determined that this incident was not preventable. Staff were in ratio with 13 children and three staff members. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with facility representative Danielle Tuttle. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
