An unannounced case management inspection was conducted today at 9:35am by Licensing Program Analyst (LPA), Laura Chavez. LPA met with Linda Sellers, Supervisor of Early Education & Instruction , in response to a self-reported Unusual Incident Report received by the Department on 10/31/2025 involving Staff #1 (S1) hitting and yelling at Child #1 (C1). Interviews were conducted with Staff #2, Staff #3 and Staff #4 between 9:48am - 11:12am regarding the incident that occurred on 10/30/2025. During today\342\200\231s inspection, classroom #1 was toured. LPA observed 4 staff supervising 18 children. Due to insufficient information at this time, further investigation is needed. All licensing reports are public information and must be made available upon request for at least three years. An exit interview was conducted, and the report was reviewed with Linda Sellers, Supervisor of Early Education & Instruction . Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
