What the inspector wrote

An unannounced case management inspection was conducted by Licensing Program Analyst (LPA) Nicolette Cunningham, who met with the head teacher to follow up on a self-reported incident received by CCL on 4/2/2026. The head teacher reported that on 4/1/2026 at approximately 9:21 a.m., Child 1 (C1) exited the front door after a parent left it open. One staff (S1) noticed C1 was missing but didn't think C1 walked out the building. C1 walked to the parking lot, where a school employee found them. Video footage showed C1 was unsupervised for approximately 3 minutes and 33 seconds. S1 stated C1 was not hurt or upset. At the time, two staff (S1-S2) were supervising 10 children. This incident constitutes a lack of supervision. The head teacher stated that a door chime was added, the door was repaired to prevent sticking, and the gate was reinforced. LPA photographed the chime, door, and gate. The head teacher met with S1 and S2 and discussed supervision during transition times and steps to prevent future incidents. The following violation of the California Code of Regulations, Tittle 22: Division 12 was observed: Absence of supervision resulting one child walking to the parking lot without supervision. See LIC 809D. *Continued on 809C LPA Cunningham informed the head teacher Diana Durham that this report dated 4/23/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Cunningham informed the facility representative to provide a copy of this licensing report dated 4/23/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the head teacher. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Notice of Site Visit shall be posted for 30 days from today's visit.

    The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based on interviews, the facility did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to children in care.
